There is no longer a legal duty to self-isolate from 24 February 2022. Applications can only be made if you have tested positive for COVID-19, been asked to self-isolate or have to look after a child before 24 February 2022. Applications must be made within 42 days of testing positive or being told to self-isolate. Anyone testing positive, told to self-isolate, or looking after a child from 24 February 2022 is no longer eligible to claim this support.

This one-off £500 payment is for people who live in the New Forest who are employed or self-employed and are having to self-isolate. To be eligible you must have to self-isolate due to:
  • having tested positive for COVID-19, or
  • having recently been in close contact with someone who has tested positive and been told to self-isolate by the NHS, and you are not fully vaccinated, or
  • having to look after a child/young person who has been told to self-isolate, and
    • tested positive for COVID-19 and is aged 15 or under, or between eighteen and a half (18 years 6 months) and twenty-five with an Education, Health and Care (EHC) plan; or
    • been identified as a close contact of someone with COVID-19 (by NHS Test and Trace, their education or care setting or the NHS Covid-19 app), is aged between eighteen and a half (18 years 6 months) and twenty-five, is not fully vaccinated and has an EHC plan, AND
    • the parent or guardian are unable to work from home and will lose income as a result.
To complete this form you will need:
  • your notification from NHS Test and Trace to self-isolate (and your 8 character NHS Test and Trace unique ID number), if you are applying for yourself, or
  • your child's details, if relevant, including confirmation from their school or childcare setting and their contact details
  • your National Insurance Number
  • your bank account details, and recent bank statements for all the accounts you have (this must include the account you want the support payment paid into)
  • if employed, proof of employment (e.g. payslip)
  • if self-employed, Self-Assessment Returns showing trading income, and proof business cannot be undertaken without social contact
  • your partners wage slips and bank statements if you are applying for a discretionary payment.
You cannot save this form part way through.

In order to qualify you must fill in this form within 42 days of the start of your self-isolation period.
